Clarify Health today introduced Clarify Arrival™, a patient activation solution that guides patients to the healthcare they need while helping the health system that serves them keep that care in network with the right providers. It covers the three ways a patient becomes a health system’s patient: the referral a physician writes, the outreach that finds someone who needs care, and the search a patient starts on their own. Those three paths are usually handed to three different vendors who never talk to each other. Arrival runs all three and counts success the same way on each of them: a booked in-network appointment, attributed to the path that produced it.

Clarify Meridian™ and Arrival are built on the same intelligence layer and are designed to work together. Meridian sees the opportunity across the entire network: where patients are leaving for care that could have been delivered in network, where access and market share gaps persist, how site-of-service decisions affect cost and quality, and how value-based contracts are performing. Arrival acts on it, converting that intelligence into patient activation, coordinated outreach, and completed in-network visits. One company, accountable from the first insight to the booked visit to the proven result.
“The moment a patient is told where to go next is the most consequential and least visible moment in American healthcare,” said Clarify Health CEO and Co-Founder Todd Gottula. “It is where a patient’s outcome is largely decided, and where a health system either continues the care it started or loses the thread of it.”
A Duke study, published in The Journal of General Internal Medicine, found that only 34.8 percent of primary care referrals resulted in a documented completed specialist appointment. Nearly two thirds did not close the loop. That translates to millions of patients every year who wait longer than they should, travel farther than they need to, or receive care from someone who was not the right match for their condition.
Consider a patient like Albert, a 60-year-old retired schoolteacher living alone with Type 2 diabetes, hypertension, and cardiovascular disease. His primary care physician recommends that he see a specialist and hands him a list of endocrinologists and their telephone numbers. Albert spends hours trying to reach the names on that list, and no one calls him back. Arrival could have made the match on his behalf, and the appointment would have been on Albert’s calendar by the end of the day. Albert’s path was a referral. Other patients may never get a referral at all, and Arrival works their paths as well.
- The referral. Meridian pinpoints the referring physicians and practices where patients are leaving the health system and identifies which specialists could have served those patients better on quality, availability, and cost. Arrival acts at the moment of the referral, presenting the right options and securing the appointment rather than handing a patient a name and a telephone number.
- Patient outreach. Meridian identifies the patients in a market who need a service line’s care and are not receiving it. Arrival reaches those patients directly, with a coordinated campaign that books the visit, so a service line with open capacity fills with the patients who most need it rather than the patients who are easiest to reach.
- The digital front door. For the patient who starts the search alone, Arrival is the front door itself: provider search built on clinically validated data, AI-powered chat, and real-time online scheduling. That capability comes from Loyal Health, which Clarify acquired in June 2026 and which close to 500 hospitals rely on today across more than 80,000 provider and location profiles.
“When my own back gave out, one telephone call collapsed weeks of uncertainty into the right surgeon and a two-week path from diagnosis to recovery,” said Gottula. “Almost no one has that telephone call. They have a searing pain, a name that came up first, and no idea how long they will wait or what quality they will find. Arrival is that telephone call, for the patient who was referred, the patient we reached first, and the patient searching alone at midnight.”

About Clarify Health
Clarify Health is an outcomes company that partners with more than 60 health systems to do the work most analytics vendors avoid: finding where the care that is best for patients and the care that sustains the health system have come apart, then working alongside the people who can close that gap, from the liaison in the field to the service line leader running the plan.
Clarify Health sees how care moves for 300 million people, priced against 5.6 trillion payer-specific rates, with quality measured at the episode level across major contracts. Clarify Meridian™ puts that view to work when a patient needs care, whether that moment arrives as a referral, as an outreach call, or as a search. The view is not what Clarify Health delivers. The outcomes are.
Healthcare at its best does not feel like a transaction. It feels like a team activated on a patient’s behalf, weeks from diagnosis to an answer instead of months, and the confidence that the people providing the care are the right people for that patient.
